El maltrato infantil y del recién nacido. La perspectiva desde Atención Primaria

El maltrato infantil (MI) es un problema importante en la sociedad actual, con consecuencias nefastas en todas las etapas de la vida del niño, siendo los menores de 5 años los más vulnerables. Es importante que el pediatra no sólo reconozca y actúe ante el mismo, sino que la prevención debe ser un pilar importante desde la concepción, siendo imprescindible la formación tanto en actitud como en aptitud para evitar su infra diagnóstico.

Los centros de salud y educativos son lugares idóneos para aplicar medidas preventivas, existiendo cuatro niveles según la población diana a la que van dirigidas, los objetivos a conseguir y las medidas a aplicar. Dentro del maltrato prenatal y del recién nacido, hay que destacar el papel de la Consulta Prenatal en nuestros centros de salud. Por otra parte, es imprescindible realizar una búsqueda activa de los factores potenciadores y compensadores, ya que la disminución de los segundos es clave para el desarrollo de cualquier tipo de maltrato. En caso de sospecha debe ser siempre notificado a través de las hojas de notificación, acompañado o no de un parte de lesiones al Juzgado, y valoración de  hospitalización del menor según sea el caso, quedando registrado en la historia clínica. 

Una vez detectado el maltrato, desde la consulta de pediatría de atención primaria, se debe vigilar las de repercusiones del mismo en el menor, haciendo hincapié en la posible existencia del maltrato reiterado, típico en la edad infantil. 

Child abuse is a major problem in today’s society, with dire consequences in all stages of the child’s life, with children under 5 years of age the most vulnerable. It is important that the pediatrician not only recognize and act before it, but prevention must be an important pillar from the conception, mabeing essential the training of pediatricians both in attitude and fitness, avoiding underdiagnosis.

The health and education centers are ideal places to apply preventive measures, there being four levels according to the target population to which they are directed, the objectives to be achieved and the measures to be applied. In prenatal and newborn abuse, we must highlight the important role of the Prenatal Consultation in our health centers. On the other hand, it is essential to perform an active search for the compensatory and enhancing factors, since the decrease of the seconds is key to the development of any type of abuse. In case of suspicion must always be notified through the notification forms, accompanied or not of a part of injuries to the Court, and assessment of hospitalization of the child as the case may be, being recorded in the Medical Record.

Once the maltreatment is detected, from primary care pediatrics consultation, it should be monitored the impact of the same on the child, emphasizing the possible existence of repeated abuse, typical in childhood.
